1. I WAS INJURED ON THE JOB. WHAT DO I DO NOW?

  • Report the injury to your supervisor immediately, but no later than 72 hours after the injury occurs.
  • Seek medical attention, if necessary. This can be done even if you don’t have a claim number assigned, just let the healthcare provider know the treatment is for a work-related injury.
  • File a Report of Injury with Workers’ Compensation within 10 days. Complete the form in its entirety, preferably with your employer. Ensure all information is correct. You must sign your report form!
  • Upon receipt, the Division will assign you a nine-digit claim number. You must give this number to all your healthcare providers so they may bill the Division directly for injury-related medical care.

2. WHERE ARE WORKERS’ COMPENSATION FIELD OFFICES LOCATED?

For the convenience of our customers, we have field offices located across the state. (Chris, link this to the Contacts page just below the main Workers’ Comp page)
